Become Active in the Healthy Start Coalition

Healthy Start’s program initiative has two distinct functions – planning and implementing.
Planning function:
Through a service delivery plan, the coalition identifies and prioritizes prenatal and infant health care needs in the county. Planning objectives are developed to address needs specific to Santa Rosa County.
Implementing function:
• Providers are identified and educated on the Healthy Start Risk Screening Program and use of the Healthy Start Prenatal and Postnatal Screening Forms.
• Care Coordination services are secured by contract with the Florida Department of Health – Santa Rosa County. The Healthy Start nurses and social workers currently offer extra support during pregnancy and the newborn’s first year of life. This is done through on-site office and home visits.
• Community outreach and awareness are provided by Healthy Start Staff, Coalition members, and Healthy Start volunteers. Healthy Start participates in community health fairs and event; provides Healthy Start outreach information in community presentations and through community partners, service agencies, and Healthy Start providers; as well as sponsoring special maternal and child health education events.
